A. DOH LTO RENEWAL
As in the past years, the Department of Health-Health Facilities and Services Regulatory Board (HFSRB) opens its doors once again for your LTO Renewal for 2025 beginning October 1 until December 15, 2024 ONLY.

1. Online renewal of LTO will still be implemented.
2. Automatic Renewal may be allowed provided the facility submits ALL required documents and has no pending sanctions or violations.
3. For Levels 2 and 3, you may still proceed with transacting with DOH CHD in your respective regions for your LTO renewals. DOH-HFSRB will issue the corresponding advisory should there be any change in the current implementation.
We are pleased to note that we are in close coordination with the DOH-HFSRB to ensure that DOH LTO's will be released PRIOR to the PHIC deadline of December 31, 2024. PLEASE SUBMIT ALL DOCUMENTS ONCE THE APPLICATION FOR LTO RENEWAL OPENS ON OCTOBER 1. Please let us know immediately should you encounter any problems with the release of your LTO so we can relay this to HFSRB.
B. PHA - PAYMENT OF ANNUAL MEMBERSHIP FEE FOR 2024 AND CERTIFICATE OF GOOD STANDING
Relative to this, we are pleased to inform you that we are now accepting PAYMENT OF ANNUAL MEMBERSHIP FEE FOR 2025 with the following applicable rates:

IMPORTANT NOTES:
1. You may already pay your AMF for 2025 now or you may do this simultaneously with your application for DOH LTO renewal which starts on OCT 01 until DEC 15, 2024 ONLY.
2. Once you have paid your AMF for 2025, please submit your DOH LTO for 2025 once available so we can IMMEDIATELY process your Certificate of Good Standing for 2025.
3. If you have not paid your AMF for 2025 prior to the release of your LTO, please ENSURE that you submit the copy of the payment transaction slip for your 2025 AMF together with the copy of your 2025 DOH LTO so we could IMMEDIATELY proceed with the processing of your CGS. WE CANNOT PROCESS YOUR CGS WITHOUT THE COPY OF THE DOH LTO AS WE NEED THE DETAILS IN YOUR 2025 LTO FOR THE CGS.
4. Once the CGS is processed, the soft copy shall be emailed to your hospital email address with the hard copy to be delivered to your hospital address via courier together with the O/R for the payment of your AMF.
5. We are also pleased to note that the system we created whereby the list of hospitals with PHA CGS is updated in real time as soon as we release the CGS to the Member Hospital, is now recognized by PHIC and included in PhilHealth Circular 2023-0021Omnibus Guidelines on the Accreditation of Health Facilities (HFs) to the National Health Insurance Program (copy attached) under C. Requirements for Accreditation. As noted, if the facility is included in the roster of Members with CGS, this will suffice and hard copies of CGS may no longer be submitted.
C. PHIC - APPLICATION FOR CONTINUOUS ACCREDITATION
1. The application for PHIC continuous accreditation is also in the 4th quarter, OCTOBER UNTIL DECEMBER 31, 2024 ONLY, no more extensions. You may start applying as soon as you have your DOH LTO and PHA CGS for 2025.
2. The PHIC Accreditation Department likewise advises to ensure that the PHIC accreditation of the Head of Hospital IS VALID at the time of your application.
3. As noted earlier, to facilitate smooth processing of your application for continued accreditation, you may already transact with your respective PRO office once the soft copy of the CGS is emailed to you.
4. Please go over and read carefully PC 2023-0021 -Omnibus Guidelines on Accreditation of Healthcare Facilities (copy attached). Copies of the fillable PROVIDER DATA RECORD and the PERFORMANCE COMMITMENT are likewise attached for your perusal.
